Transcranialdoppler and stroke: a review of literature in sickle cell disease

This study aimed to evaluate, through literature of three databases: Medline, Lilacs and
SciELO during the period from 2008 to 2013, the effectiveness of Transcranial Doppler (TCD) to
prevent cerebrovascular accident (CVA) in Sickle cell disease (SCD) patients. After analysis of the
literature it was observed that most studies were limited to HbSS and β-thalassemia genotypes in
populations under 20 years old. The results demonstrated that stroke can be avoided by TCD
screening. It was efficient in the prevention of strokes, reducing its incidence. Children with the
HbSS genotype tend to have an increased TCD velocities in all brain blood vessels compared to
those in the same age without the disease, which is a significant risk factor for stroke. It was
concluded that TCD is a predictive test for rating the risk of stroke, however there are challenges
yet to overcome
